Patient Checklist:  Please bring the following items with you on the day of your appointment.

• Your insurance co-pay.
   
• All medical and vision insurance cards.
   
• Medical History form completed.
   
• Patient Information form completed and signed.
   
• Complete list of all medication you are currently taking.
   
• If patient is a minor, a parent or legal guardian must be present at the initial visit.
   
• If your insurance plan states the need for a referral from your primary care physician please remember this is your responsibility.  Each visit may require a separate referral.
